Following her apparent drug overdose last month, a source tells ET that the 25-year-old singer has left the Cedars-Sinai Medical Center after almost two weeks in the Los Angeles hospital, and has entered rehab for further treatment.

The "Skyscraper" singer said her addiction is "something I must continue to overcome and have not done yet", as she took to Instagram Sunday afternoon.

"I will keep fighting ❤️", Demi concluded.

Demi was rushed to hospital on July 24 after being found unresponsive at her home in the Hollywood Hills. The paramedics reportedly administered Narcan, an antidote used for opioid overdoses. She had been sober for six years from alcohol, cocaine and OxyContin, according to TMZ. She wants to get help. News also confirmed that Demi has been discharged and is in rehab.
